The universal questionnaire of specimen acquisition was adopted and used. It composed of two categories; the first category is general information i.e. type of hospital, numbers of bed, numbers of daily specimen, experienced on performing blood collection, academic background, the providing of guideline or standard operating procedure (SOP) for blood collection, numbers of daily blood collection, and numbers of accident or near-miss events during blood collection; the other category is several questions related to the performing of blood collection (Garza D, et al. 1996). This questionnaire was validated by the peer group. Evidence-based information retrieved from participants was analyzed, synthesized and used as tool of learning.
