Corporate HES recently released the 2013 Major Incident Study(MIS) which provides an analysis of serious injuries and fatalities, major fires, large spills, toxic releases, major motor vehicle crashes, significant near misses and high cost reliability incidents that occurred in our global operations in 2013. This latest study identifies similar root causes that we have seen in previous Major Incident Studies; however, Leadership Accountability was identified as a more frequently cited root cause in MIS 2013. Please review the MIS conclusions and feel free to pick 12 associated case studies as OE moment for your team discussion. The deployment package will be provided through the upcoming Leadership Talk.
