1. MINUTES OF PREVIOUS The minutes were confirmed and accepted without amendment
2. OHS Objectives & Targets.
1. Steps to maintain Fatal Accident To Zero
2. To Maintain no. of reportable industrial accident /incident not more than 2 numbers /project
3. To maintain OHS Complaints not more than 2 numbers /month/project.
4. Accident frequency rate shall be < 2 per million man-hours
5. Accident severity rate shall be less than < 100 million man- hours
3. OPENING ADDRESS
Safety Advisor Edde opened the meeting by advising the members of the committee to be more cautioned of when working on the A frame ladder as there were 02 cases of incident of workers falling off the ladder while working on them.
Also reminded the committee that the project is entering the last final stages of finishing and there will be a lot of pre commissioning & pressure testing works to be carried out. All contractors were advised to follow all MS/RA / LOTO approved by Hitachi to carry out such work activity and also not to leave and loose material inside any pipes.
Project Control Manager Mihara San also advised the committee to do regularly do their housekeeping after completion of their job activity and clear small bits and pieces of cable ties and debris to be swept clean.
4. MATTERS ARISING
4.1. Housekeeping.(Item 8.1)
TCC & CHUBB immediately did their housekeeping at their fabrication work area before they begin their daily work activity the very next day.
5. WSH COMMITTEE INSPECTION
5.1. Follow Up on May WSH Committee Inspection
The 07 items had been rectified.
5.2. JUNE WSH Committee Inspection Report
04 Safety deviations found were as follows:
a) Loose materials along scaffolds platforms at height.
b) Obstruction of access way at area 7.(Chubb)
c) Use of cargo lift area 7(TCC )
d) Electrical cable laid on the ground Area 7 (TCC)
All contractors were directed to rectify the above immediately after the meeting for all those items still left due to time to rectify the items.
6. SITE SAFETY INSPECTION
6.1. May Hitachi Inspections Reports
8 deviation (unsafe conditions & unsafe acts) were found. Below chart showing the various types of deviation types:-
The number of deviations has decreased from April 18 nos to May 8 nos.
There has been a decrease of work at height violation from 4 to 3 from the previous month of April. All contractors are advised to further educated and do their daily JSA briefing to their workers on their work at height requirements to further reduce the violation to achieve ZERO work at height violation as our target and goal for the coming months. Together we will be able to achieve this.
All contractors are advised to review their Risk Assessment and safe work procedure and full time supervision of the immediate supervisors will help to reduce the areas of concern such as Work at Height violation which could result in a fatality if unnoticed.
With the above Hitachi expect the number of deviations to decrease even further for an even safer and healthier workplace for all.
Deviations of Concern
1) Work At Height. (100% tie off at all times/ improper working platform.)
Countermeasures for deviations of concern
Sub- contractors were directed to ensure corrective actions to be taken to prevent such above deviation from happening again by reviewing their safe work procedure, Risk Assessment and full time supervision at site by immediate Work At Height supervisor of each contractor.
7. SAFETY PERFORMANCE
7.1. Occurrence : Month of April
Accident : 0 Incident : 0 Near-Missed : 0 Occurrence : 0

7.3. Safety Deviation ( Non Compliance ) Statistics
All Contractors were advised to reduce the amount of deviations and keep up the good work attitude and to reduce the deviation further by compliance and close monitoring of its supervisors.
8. WORK ACTIVITIES IN MAY
Subcontractors informed their work activities and HTC issued reminders that all work activities must carry out the followings:-
• Establish a work plan for each work activities.
• All work activity to be done in accordance to approved MS/RA.
• Carry out risk assessment for each work to be executed.
• Ensure safe work practices and procedures are followed.
• Comply with HTC permit-to-work system and all the conditions in permit.
• Comply with lifting operation plan and code of practices.
• Comply with statutory requirements and code of practices.
• Conduct Tools Box Meeting using the RA for the work.
• Obtain approval from HTC Discipline Supervisor before starting work.
• Subcontractor supervisor to supervise and check for safety before, during and after work.
HTC emphasized the important of complying with the above. It was agreed that work will be stopped if the any non compliance discovered by Hitachi. In addition, disciplinary action will be taken against the supervisor in charge of the work.
Furthermore subcontractor will be responsible, accountable and liable for any accident and injury to any one.
8.1. SMCC Work Activities

Safety Reminders
a) Keep clear of the point of operation and work area.
b) All work at height activity to follow 100% tie off.
c) Pinch Point when covering of drainage.
8.2. TCC Work Activities
1) Cutting & grinding of pipes(fabrication)
2) NDT works
3) Insulation & Painting
4) Pressure testing
Safety Reminders
a) Daily check on all gas cylinders before start of hot work activity.
b) To always make hot work area free from combustible materials.
c) Follow approved MS/RA for all work activity.
d) All pressure testing work area to be displayed of warning signage’s and barricaded with yellow black tapes at all times.
8.3. SDS Work Activities
1) Cable pulling.
2) Cable termination.
3) Motor Hyper testing.
Safety Reminders
a) Regular break time for workers to reduce work fatigue while cable pulling.
b) All test areas to be barricade and displayed with proper signage.
8.4. AKTS Work Activities
1) Cable tray installation
2) Instrument Air Supply pipe installation.
3) Electrical equipment installation.
4) Cable laying/pulling.
5) Cable testing & termination works

a) Follow buddy system when working on A frame ladders
b) 100% tie off when working on pipe racks for installation.
c) Follow all Work At height regulations.
d) All test areas to be barricade and displayed with proper signage.
8.5. CHUBB Work Activities
1) Sprinkler Pipe installation
2) Pipe fabrication at workshop.
3) Fire alarm cable & conduit pipe installation.
4) Pressure Testing works.

b) Banks man to guide all M.E.W.P.
c) All hot work procedure to be complied at all times at fabrication workshop
9. Any Other Matters
No matters discussed to be followed up to the previous meeting.
As there were no other matters to discuss, meeting adjourned at 11.45 a.m.
