Dear Colleagues,
Within a month, Chevron Thailand has experienced two serious incidents resulting in two Days Away From Work Cases (DAFWC). These two cases were routine jobs but high risk, particularly the second one, which was similar to lessons learned on Level 2 Near Miss cases back in August 2012 and July 2009. This is a strong reminder to you that we always have risks in our daily operations and you should never let your safety guard down. I would like to re-emphasize on your commitment to “Always” follow Tenets of Operation and exercise Stop Work Authority without delay when unsafe acts or conditions. Please look out for yourself and your co-workers on the job.
As earlier communicated, I emphasized each of you to remain focused on our operational discipline and realign our efforts to verify that job safety analysis is effectively conducted prior to each job, all risks are identified and all required safeguards are in place and effective in mitigating those risks. You must be alert to all potential risks, ensure mitigation measures are in place, and be diligent in doing every task the right way, every time. I would also encourage all field management and supervisors to increase oversight and effectively conduct field verification and validation.
I personally believe that every incident is preventable and zero is achievable. With your clear focus on safety and strong commitment to “Always,” you and every member of our workforce can return home safely every day, ZERO is more than just a Chevron goal. Your family, friends and co-workers are counting on you!
In closing, the injured persons involved in these two DAFWCs have been receiving good medical care and are recovering. There were many lessons to be learned from these serious incidents and they will be shared across our operations once the incident investigation is completed.
