ALCOHOL PROPOSAL FORM and ALCOHOL GUIDELINES
For events sponsored by recognized MIT students and student organizations in space maintained by the Campus Activities Complex, DAPER, or Academics/Scheduling Office.
This form has been created to aid students and students groups in developing a proposal for events where alcohol will be served. Student groups must write-up and attach a proposal letter to this form, and then submit it to the Student Activities Office in order to obtain approval to serve alcohol.
Once completed, proposals should be submitted with an Event Registration Form to the Student Activities Office in W20-549. In addition, the Event Host must read and sign-off on the Alcohol Guidelines as outlined on the back of this form. If alcohol will be served at your event, please allow at least an extra week of planning time to obtain the proper approvals.
Things to keep in mind: Wristbands are required for all events. Standard MIT policy for events with alcohol requires a 3rd party vendor cash bar with only
