5.1 Nonselective nonsteroidal anti-inflammatory drugs, COX-2 inhibitors, and
acetaminophen
Nonsteroidal anti-inflammatory drugs (NSAIDs) form a heterogeneous group of organic
acids that have analgesic, antipyretic, anti-inflammatory, and platelet inhibitory actions.
NSAIDs have long been used in the treatment of nonoperative pain syndromes; their
mechanism of action is through the nonspecific inhibition of COX, which therefore blocks
both the constitutive COX-1 isoform, responsible for gastric protection and platelet function,
and the inducible proinflamatory isoform, COX-2. Thus, the actions of the NSAIDs result in
both the desired analgesic effects and the unwanted adverse effects of the COX isoforms.
