Care management – the heart of integrated careIn policies concerning integrated care for the disabledelderly, care management has become an importanttopic for consideration. Standardised care, continuityof care, flexibility of care, and finely tuned co-ordinationbetween the different kinds of care providers area central part of care management in order to realisehigh quality care for the disabled aged and to enablethem to continue to live independently in their ownhomes for as long as possible. This is the mostimportant reason that the Japanese LTCI scheme hasformalised the care management process. A caremanager is entrusted with the entire responsibility ofplanning all care and services for individual clients.According to the results of a needs assessment of theclient and his or her wish, a care plan is drawn up.The care manager organises the care specified in thecare plan and works with the client, supervises andevaluates the care process (monitoring). When necessary,the care plan is adjusted. In this way the caremanager plays a pivotal role in the LTCI scheme. TheLTCI scheme pays a fee of care management foreach care manager
