At 15:10hrs, a cement hose was being lifted from the supply boat to be placed in
a saddle rack on the starboard side hang off point. This operation is considered
to be a routine lift. The IP and helper were situated on the walkway behind the
hang off point, in close proximity to suspended load. The hose saddle/lifting sling
tie off point was suspended approximately 10m up in the air (to allow crew to
hold bottom of hose with a rope, so that the hose can be lowered on to the
saddle). While the Crane Operator was maneuvering the bulk hose to the hang
off saddle, the wire lifting sling connected to the bulk hose clamp parted. The
bulk hose fell onto the walk way and across the engine exhausts striking the IP
on the left head temporal of the face. His helper managed to get out of the way
and was not injured. IP was sent to medic for evaluation. Medic reported small
graze and swelling to left head temporal. Operations was stopped, and 1 MT
parted sling was replaced with new 3 MT wire rope sling.
